IL-1120-V

This article explains what Illinois Form IL-1120-V is and gives precise, line-by-line instructions to complete and file it.

Form IL-1120-V is the Illinois Department of Revenue’s payment voucher that corporations use to mail a check or money order for Corporation Income and Replacement Tax. It is not an income tax return; it simply matches your payment to the correct corporation and tax year. The voucher prints the state’s mailing address and includes labeled boxes for your FEIN, legal name, mailing address, phone, tax-year ending month and year, and the payment amount in whole dollars only. If you pay electronically (for example, through the Department’s online system or the QR code) you do not file this voucher at all. And if the Department has told you that you’re required to pay electronically, you must use an electronic option rather than this paper voucher. When paying by mail, you write your FEIN, tax-year ending, and “IL-1120-V” on the check or money order, make it payable to Illinois Department of Revenue, detach the voucher, and mail both to the PO Box shown on the form. If you’re also mailing your corporate return, you may attach the voucher and payment to the front page of the return and mail them together as directed on the return. The voucher can be used for extension, estimate, or return payments when you are not paying electronically.

How To File It

  1. Decide Your Payment Method.
    • Electronic payment: Strongly encouraged; you’ll avoid mailing delays, skip the post office, and receive immediate confirmation. Use the options at tax.illinois.gov or scan the QR code shown on the form. If the Department has notified you to pay electronically, you must use an e-payment option.
    • Paper check or money order: Use Form IL-1120-V and mail your payment.
  2. Know When Not To File The Voucher.
    • If no payment is due, do not file IL-1120-V.
    • If you pay electronically, do not file IL-1120-V.
  3. Mailing Address For Voucher Payments.
Mail the detached voucher with your check or money order to:
Illinois Department of Revenue, P.O. Box 19038, Springfield, IL 62794-9038.
