Instructions For The Header Section
Legal Name

FEIN/SSN
Provide your Federal Employer Identification Number (FEIN). If you are a sole proprietor and do not have an FEIN, enter your Social Security Number (SSN).
IRP Account
If you already have an existing IRP account number with Indiana, enter it here. If you are a new applicant, you may leave this blank or mark it as “New.”
Fleet
Enter the specific Fleet Number assigned to the group of vehicles you are registering.
Staggered Month
Indicate the renewal month assigned to your fleet (the month your registration expires).
