Lines 1-7: Contact Details

Line 8: Federal Employer Identification Number
Provide your Federal Employer Identification Number (FEIN). If you are a sole proprietor and are not required by federal law to have an FEIN, you must enter your Social Security Number in this space instead.

You must use Form M-219 to secure this bond, and it must remain valid for the entire calendar year.
