When To Use New York IT-201-V

Use Form IT-201-V when you owe New York State income tax and are paying by check or money order.

Do not use this form to pay a tax bill, notice, or collection letter from the Tax Department. If you received a bill, follow the payment instructions on that bill.

Do not use this form to request an installment payment agreement. If you want an installment agreement for a bill you received, use the proper installment agreement process instead.

How To Prepare Your Check Or Money Order

Check Or Money Order Payee: Make the check or money order payable to New York State Income Tax.

Payment Currency: The payment must be in U.S. funds.

Memo Information: Write the last four digits of your Social Security number, the tax year, and Income Tax on the check or money order.

Attachment Rule: Do not staple, tape, or clip the payment to Form IT-201-V. Keep it loose in the envelope.

How To Complete New York IT-201-V Form

Voucher Section

Tax Year: Enter the tax year for the income tax return connected to the payment. Use the four-digit year format, such as 2025.

Your First Name And Middle Initial: Enter your first name and middle initial exactly as they appear on your income tax return.

Your Last Name: Enter your last name. If you are filing a joint return, your spouse’s name is entered on the spouse line below.

Payment Amount, Dollars: Enter the dollar amount of the payment you are sending.

Important Payment Notes

Electronic Payment Reminder: The form encourages taxpayers to pay electronically because it is faster and can be scheduled up to the due date.

Returned Payment Fee: If a check, money order, or electronic payment is returned by a bank for nonpayment, the Tax Department may charge a $50 fee.

Bank Or Department Error Exception: If an electronic payment is returned because of an error by the bank or the Tax Department, the $50 returned payment fee is not charged.

Separate Returned Payment Bill: If a returned payment fee applies, the Tax Department sends a separate bill for each returned payment connected to a return or tax document.
